What Is the Vagus Nerve—and Why Is It Important?
The vagus nerve is a major cranial nerve and part of the parasympathetic nervous system that controls:
- Movement of the vocal folds, including voicing, breathing, and triggering coughing
- Muscles used in swallowing
- Digestion and heart rate regulation
- Mucus production in the airways and gastrointestinal tract
- Parts of the body’s inflammation and immune response
Located near the head and neck, the vagus nerve can be damaged during surgical tumor resection, exposed to radiation, or susceptible to chemotherapy-induced peripheral neuropathy, especially in cancers involving the pharynx, larynx, or thyroid gland.
Dysphagia and the Vagus Nerve
Swallowing may seem like a simple reflex, but it’s actually a coordinated effort involving multiple nerves and muscle groups, many of which are controlled by the vagus nerve. This nerve plays a central role in managing:
- The pharyngeal phase of swallowing (moving food from the throat to the esophagus)
- Laryngeal (voice box) elevation and closure to protect the airway
- The upper esophageal sphincter, which opens to allow food into to the stomach
When the vagus nerve is damaged—due to surgery, radiation, chemo or tumor pressure—patients may experience:
- Dysphagia (difficulty swallowing)
- Aspiration (food/liquid entering the lungs)
- A sensation of food getting “stuck”
- Hoarseness or changes in voice if the vocal cords are also affected
In some cases, repeated aspiration can lead to respiratory infections or chronic lung changes. This is why early referral to speech-language pathology (SLP) is so important: targeted therapy and instrumental swallowing studies (like MBSS or FEES) can help evaluate function and guide safer eating strategies.
How Does Damage to the Vagus Nerve Affect Speech and Breathing?
Vocal fold paralysis can result from vagus nerve impairment. In some cases, vocal cord issues occur gradually, especially when fibrosis (a stiffening of tissues) develops over time due to radiation therapy. When one or both vocal cords lose mobility:
- Airflow through the airway can be restricted
- The voice may become weak, breathy, or lost entirely
- Cough reflex weakens, making it harder to clear mucus or sense when food or liquid is going “down the wrong pipe”
- Breathing may require the placement of a tracheostomy tube if vocal folds remain closed or nearly closed.
These effects can impact speech, swallowing and breathing and may require ongoing support from an otolaryngologist (ENT), speech language pathologist (SLP), respiratory therapist (RT) and/or pulmonologist.
Why Humidification Matters for Tracheostomy Patients

Breathing through a trach bypasses the nose and upper airway, where air is typically warmed and moisturized. Without this natural filtration, the lungs may experience:
- Dryness and irritation
- Excessively thick mucus
- Increased bacterial growth risk
- Difficulty clearing secretions
- Increased coughing
Humidification devices and filters help regulate airflow and promote healthier lung conditions for trach users. Some trach systems incorporate both speaking valves and humidifiers, giving patients more freedom to communicate while keeping mucus manageable.
