What is Nasopharyngeal Cancer?
Nasopharyngeal cancer (NPC) is a rare type of cancer that starts in the nasopharynx, or the upper part of the throat, behind the nose, and at the base of the skull. It’s more common in places like Southeast Asia and is often linked to the Epstein-Barr Virus (EBV). Symptoms can include a lump in the neck, nosebleeds, hearing loss, or headaches. However, these symptoms are often vague, so many people don’t realize something is wrong until the cancer is more advanced. Most cases are found in people between 45 and 59 years old. Treatment for NPC usually involves radiation, chemotherapy, or a combination of the two.
Finding NPC early can make a huge difference by improving the chances of survival. A recent study showed that using a blood test for EBV can help find NPC earlier, which could save lives. If it’s found and treated early, about 95% of patients live at least five more years. But if it’s found late, survival drops to about 50%. This is why finding better ways to detect NPC earlier is so important and has driven scientists to find minimally invasive tests to screen for levels of EBV.

How Epstein-Barr Virus (EBV) Helps in Screening
EBV is a common virus that spreads through saliva, blood, or other bodily fluids. Most people get EBV at some point in their lives, but it usually doesn’t cause serious problems. However, in some cases, EBV can cause cells in the nasopharynx to grow abnormally, which increases the risk of cancer. (It is important to note that most people infected with EBV will NOT go on to develop cancer.)
Identifying an EBV infection can be challenging because its symptoms often mimic those of other illnesses, like a cold or sinus infection. When you get exposed to EBV, your body creates special proteins called antibodies to fight off the virus. A recent study used a simple blood test in nearly 175,000 people over 12 years to look for those antibodies, and grouped the patients into high-risk, medium- risk, or low-risk categories. Those at high risk had further testing, like a nasal endoscopy or a biopsy, to see if they had cancer. Those at high risk without cancer, and those with medium risk had regular check-ups to help monitor their cancer risk going forward.

- Finding Cancer Early: Screening helped catch cancer at an earlier stage. About 30.5% of cancers found in the screened group were in early stages (I or II), compared to 20.5% in those who didn’t get screened.
- Better Survival Rates: People whose cancer was found through screening were less likely to die from NPC. The screening program lowered the risk of dying from NPC by 54%.
- Accurate Testing: The screening did not lead to more false positives or overdiagnosis. This means it’s good at finding real cases of cancer without causing unnecessary worry.
What Can We Do Next?
Experts suggest making NPC screening easier and more available. For example, people could collect their own blood samples at home or get tested in their doctor’s office. For those at high risk, using simpler follow-up tests could make the process less invasive. It’s also important to teach people about NPC and why screening is helpful so that more people participate. Doctors and patients can work together to decide if screening is a good idea for each person.
Using EBV-based blood tests to find NPC early could make a big difference in reducing deaths from this disease. By helping more people get screened and making the process simple, we can improve public health and save lives.
